Problem: The products may be contaminated with Listeria monocytogenes.

Discovered: A FSIS microbiological sample collected on 06-25-2001, returned positive results for Listeria monocytogenes on 07-01-2001.

Discovered: FSIS collected a microbiological residue surveillance sample on April 20, 2001, that returned positive results for penicillin, an antibiotic, and sulfadimethoxine, an antimicrobial, on June 01, 2001. The product represented by the sample was used as an ingredient in the production of the above-mentioned products.

Nutramigen® 16-OZ. Powder Infant Formula and  Nutramigen® 32-OZ. Ready-To-Use Infant Formula Recalled Due To Spanish-Language Labeling Error That Could Result in Adverse Health Affects, Product is Safe When Prepared Correctly

 

Mead Johnson Nutritionals is recalling certain batches of Nutramigen® 16-oz. powder infant formula and Nutramigen® 32-oz. ready-to-use infant formula because the Spanish-language side of the label for both products gives incorrect preparation instructions. The English-language instructions are correct and the product is safe to use as directed in English. If not properly prepared, Nutramigen® 16-oz. powder infant formula and Nutramigen® 32-oz. ready-to-use infant formula have the potential to cause serious adverse health effects such as seizures, irregular heart beat, renal failure or in extreme cases, death. Symptoms to look for include vomiting, diarrhea, decreased urine output, irritability, decreased activity or sunken eyes.

If your infant has experienced any of these symptoms while consuming either of these products contact your pediatrician or health care provider. Infants living in hot climates, as well as those with fever, pre-existing diarrhea, or chronic illness are at increased risk for life-threatening complications.

 

The affected Nutramigen 16-oz. powder product includes the following batch codes: BHC43, BHC44, BJC45, BJC46, BJC47, BJC48, BKC49, BKC50, BLC51, BLC52, BLC53, BMC54, BMC55, BMC56, BAM57, BAM58, BBM59, BBM60, BBM61, BCM62, BCM63, BCM64, BDM65, BDM66, BEM67, BEM68, BEM69, BEM70, BEM71, BEM72, BFM73, and BFM74. The batch codes are embossed on the bottom of each can.

The affected Nutramigen 32-oz. ready-to-use product includes the following batch codes: MBM90, MBM91, MCM92, MCM93, MCM94, MDM95, MDM96, MEM97, MFM00 and MFM01. The batch codes are embossed on the top of each can.

Both Nutramigen product forms, 16-oz. powder and 32-oz. ready-to-use, were distributed nationwide, as well as in Guam, the Dominican Republic and Puerto Rico.

 

Consumers who have cans of product from these batches may return the product to the place of purchase for a full refund or use the following preparation instructions. BelGioioso Cheese, Inc. of Wilmington, CA is recalling 300 pounds of its 8-ounce bags of Fresh Mozzarella packed in water because of contamination with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women. The recalled bags were distributed in Southern California in retail stores.

The product comes in an 8-ounce, printed package marked with an expiration date of 7/16/01 printed on the package.

The contamination was noted after routine testing by the Food and Drug Administration revealed the presence of Listeria monocytogenes. This recall is being conducted with the knowledge of FDA. Consumers who have purchased 8-ounce bags of Fresh Mozzarella with a 7/16/01 code date are urged to return them to the place of purchase for a full refund. Publix Super Markets Inc. is voluntarily recalling Publix Premium Coffee Almond Fudge Light Ice Cream half gallon carton sell-by date: 15 NOV 01, 12-444. This product may contain undeclared peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of serious or life threatening allergic reaction if they consume these products.

Publix Spokesperson Lee Brunson said, "Through our internal quality assurance process we were able to recover most of the affected product. Although only 216 cartons were shipped to a limited area of stores, as a precaution we are recalling all product within this sell-by date purchased in Florida, Georgia, South Carolina and Alabama."

Publix is encouraging all customers who purchased the affected product to return the product to their store for a full refund or replacement. The company has received one complaint regarding this product.

 

Product: "MR. MEAT" brand "MEAT BALLS HAMBURGER" in 10 lb. cases, "MR. MEAT" brand "MEAT BALLS GROUND BEEF" in 10 lb. cases, "MR. MEAT" brand "HAMBURGER" in 20 lb. cases and "MR. MEAT" brand "GROUND BEEF" in 5, 6, 7.5, 10, and 20 lb. cases.

Codes: Produced from June 15, 2001 through July 13, 2001. Each case bears a date code ranging from "0615" to "0713," and "EST. 2559" appears on the label inside the USDA seal of inspection.

Problem: The products may contain soy grits that are not declared on the label. Soy is a Class II allergen.

Discovered: FSIS collected product samples on July 9, 2001 which returned positive results for soy on July 12, 2001.

Fortuner's Food Co., Inc. is recalling 4,000 packages of Dried Lily Flower because the product contains undeclared sulfites.People who have an allergy or severe sensitivity to sulfites run the risk of serious or life threatening allergic reaction if they consume this product. No illnesses have been reported to date in connection with this problem.

 

Product was distributed in the New York City area. Fortuner's Dried Lily Flower is distributed in a uncoded, heat sealed 6 oz. plastic bag.

The recall was initiated after it was discovered through routine sampling by NYS Dept. of Agriculture and Markets that the product contained sulfites and was distributed in packaging that did not declare the presence of sulfites. The consumption of 10 milligrams of sulfites per serving has been reported to elicit severe reactions in some asthmatics. Anaphylactic shock can occur in certain sulfite sensitive individuals upon ingesting 10 milligrams or more of sulfites. Analysis of the Dried Lily Flower revealed that it contained 84.5 milligrams per serving.

ATF Fitness Products, Inc. is recalling BioPharm brand T-Cuts, a dietary supplement, because it contains tiratricol, also known as triiodothyroacetic acid or TRIAC, a potent thyroid hormone that may cause serious health consequences including heart attacks and strokes.

 

The T-Cuts was distributed nationwide to retail stores or mail order. T-Cuts is packaged in 90-count capsules in white bottles with black lettering. All lot numbers are affected by this recall. No illnesses have been reported to date.

ATF Fitness Products, Inc. urges all individuals who may have purchased this product, or any product containing tiratricol, to stop using them immediately and, in addition, to consult their health professional if they are experiencing any adverse effects, which may include insomnia, nervousness, sweating, and diarrhea. Sargento Foods Inc. of Plymouth, Wisconsin is voluntarily recalling 8-ounce packages of Sargento Fancy Shredded Mozzarella with code dates 19OCT01 and 20OCT01 and Sargento 6 Cheese Italian Blend Shredded Cheese with code date 20OCT01 because they have the potential to be contaminated with listeria monocytogenes.  The recalled products were distributed nationally in retail stores. Consumers who have purchased 8-ounce packages of Fancy Shredded Mozzarella or 6 Cheese Italian Blend Shredded Cheese with the above-noted code dates should return the products to the store where they were purchased for a full refund.

Eastland Food Corporation of Columbia, Maryland, a distributor of Asian foods in the eastern United States, is immediately recalling 12 ounce jars of NEWFOODS BRAND MACAPUNO STRINGS and NEWFOODS BRAND SWEET SUGAR PALM because government testing indicates they may contain excessive levels of sulfites which are not listed on the product labels. People who have an allergy or severe sensitivity to sulfites run the risk of serious or life-threatening allergic reaction if they consume these products.

The recalled NEWFOODS BRAND MACAPUNO STRINGS and NEWFOODS BRAND SWEET SUGAR PALM were distributed by Eastland Food Corporation throughout the eastern United States primarily to supermarkets and grocery stores which sell Asian food products.

 

Both products come in 12 ounce, clear glass jars with white metal tops. Macapunos are a type of coconut found mainly in the Philippines. Macapuno strings from the coconut are used in a variety of desserts. Sweet Sugar Palm is a type of Asian fruit eaten primarily as a dessert.

The recall was initiated after a government testing authority notified Eastland Food Corporation, one of the distributors of NEWFOODS BRAND MACAPUNO STRINGS, that the product it tested contained 10.9 milligrams of sulfites per serving. According to health authorities, sulfites in excess of 10 milligrams per serving pose a severe hazard to sulfite sensitive individuals and can be life threatening to these individuals. The testing also indicated that NEWFOODS BRAND SWEET SUGAR PALM contained 7.7 milligrams of sulfites per serving.

NEWFOODS INTERNATIONAL CORPORATION, the Philippines based manufacturer of dessert products, has joined in this recall but has not confirmed the results of the government testing.

 

Consumers who have purchased 12 ounce jars of these products are urged to return them to the store from which they were purchased. Since the product may also have been distributed by other wholesalers, Eastland Food Corporation, cannot assure refunds on all returns. Mon Chong Loong Trading Corp. is recalling all of its Chinese Herb Soup, because it contains undeclared sulfites. People who have an allergy or severe sensitivity to sulfites run the risk of serious or life-threatening allergic reaction if they consume these products. It is packaged in plastic bags with code: 2000.11.20 and is labeled in Chinese.

 

Chinese Herb Soup was distributed in the New York metropolitan area through retail stores. The recall was initiated after routine sampling by Food Safety Inspectors of the New York State Department of Agriculture and Markets revealed the presence of sulfites in the product not declared on the label.

 

Consumers who have purchased Chinese Herb Soup are urged to return it to the place of purchase for a full refund. Consumers with questions may contact the company at 718-417-1668.

 

 

Mead Johnson Nutritionals is requesting that consumers who may have purchased LactoFree® infant formula sample packs containing two 3 oz. ready-to-use Nursette® bottles and two Easy One® single serve powder packets return the sample pack to the place of purchase for a full refund. The six sample packs in question do not list ingredients on the back of the box. Infants allergic to milk protein run the risk of a serious or life threatening allergic reaction if they consume this product. There have been no reports to Mead Johnson from consumers or health care professionals about any infants who have become ill as a result of this labeling error. No other sizes or forms of LactoFree are affected.

 

The LactoFree sample packs were among products shipped to thirty stores in the following nine states: Colorado, Iowa, Kansas, Louisiana, Minnesota, Missouri, Oklahoma, Tennessee and Texas.
