present a serious health hazard to consumers. These herbs contain aristolochic
acid. The herb Akebia Trifoliata Caulis (Mu Tong) and the herb Asarum Sieboldii
Herba cum Radix (Xi Xin) were part of the formula for products listed below and
distributed nationwide to medical doctors, naturopathic physicians, licensed
acupuncturists, and herbal practitioners. The products were labeled under the
following brands and includes all lots and production codes:

discontinue their use and return them to the place of purchase for a full

Mead
Johnson
Nutritionals is requesting that physicians temporarily discontinue the distribution
of LactoFree® infant formula sample packs
and Enfamil® A.R.® infant formula sample packs
because they do not list ingredients on the back of the box. Although there is
nothing wrong with these products, consumers who received these sample packs are
being asked to consult with their physician before using these products.
Infants allergic to milk protein run the risk of a serious or life threatening
allergic reaction if they consume these products.
The
LactoFree infant formula sample packs contain two 3 oz. ready-to-use Nursette® bottles and two Easy One® single serve powder
packets. The Enfamil A.R. sample packs contain three 3 oz. ready-to-use
Nursette bottles and a single hole nipple. No other sizes or forms of LactoFree
or Enfamil A.R. are affected.
The
LactoFree and Enfamil A.R. sample packs have been distributed to physicians
nationwide. There have been no reports to Mead Johnson from consumers or
physicians about any infants who have become ill as a result of this labeling
issue.

Napoli is
recalling product labeled as Italian Rolls because it may contain undeclared
eggs. Dinner rolls, which contain eggs were inadvertently packaged in bags and
labeled as Italian Rolls, which do not contain eggs. People who have an allergy
or severe sensitivity to eggs run the risk of serious or life-threatening
allergic reaction if they consume this product.
Rolls
were distributed through Price Chopper stores in Troy, Latham Cohoes and
Watervliet. Product is packaged in a 16 oz., uncoded clear plastic bag,
containing twelve rolls. The recall was initiated after it was discovered by
New York State Department of Agriculture and Markets lab analysis that the
dinner rolls contain eggs.

Supplement Capsules sold in 90 and 180 count bottles. The product contains 1000 mcg of tiratricol (100 mg of
Tricol). The product is sold 12 bottles

product on the market at the time the recall was initiated.
REASON: The product contained tiratricol (also known as
triiodothyroacetic acid or TRIAC), an active thyroid hormone. The product’s label recommends to not exceed
three capsules daily. This dose is in
excess of the usual replacement dose of thyroid hormone and therefore would
cause hyperthyroidism. Particularly in patients with underlying cardiac
disease, this poses a significant health risk because of immediate,
life-threatening consequences as heart attack, stroke, heart failure and
arrhythmia.

digestive aids produced by Biotics Research Corporation of Rosenberg,
Texas and distributed nationally are being recalled because of potential
salmonella contamination. The recall involves three shipments of Hydrozyme in
90 and 250 tablet size bottles and one shipment of HCL-Plus in 90 tablet
bottles.
The recalled products were distributed between Feb. 19 and April 25. They are Hydrozyme lot numbers 21173, 21245 and 21280 and HCL-Plus lot number 21150. The recall was initiated after routine testing found salmonella in some samples of pepsin, the raw material used in the products, FDA said. semi-soft cheese, a product manufactured in Germany and distributed nationwide
in the United States, because the "egg white lysozyme" was not
specified in the food ingredient list. On the ingredient lists, the word
"enzymes" was used rather than specifically stating "egg white
lysozymes," which are enzymes derived from eggs. People who have an
allergy or severe sensitivity to eggs run the risk of serious or life threatening
allergic reaction if they consume this product. Other than to egg-sensitive
consumers, the product being recalled is safe for consumption.
The product was sold in random weight chunks between 7 and 9 ounces in deli cases at retail stores. The recall only affects packages whose label does not state "egg white lysozymes" on the ingredient list. Starting on June 26, 2001, these products were removed from the shelf and relabeled. However, if you purchased this product before the relabeling was done, the ingredient list will be in error.

Bareman
Dairy today
announced that, in cooperation with the U.S. Food and Drug Administration
(FDA), it is conducting a recall of 19,700 Gallons of Bareman Dairy Crystal
Clear Drinking Water Packaged in One Gallon Containers. The Gallon Containers
are coded will a “sell by” date of Dec 8 JD. Bareman Dairy has determined that
a portion of this lot may contain a equipment sanitizer. This sanitizer is made
of a mix of peroxiacetic acid and hydrogen peroxide. Consumption may cause
irritation or burning of the throat and stomach. If you experience irritation
or burning, immediately dilute by drinking several glasses of water and contact
the proper medical authority, do not induce vomiting. The recall is being
instituted after company testing pursuant to a customer complaint received at
10:00AM Thursday August 16, 2001. The product, run under the Bareman Dairy
label, is distributed in Michigan’s Lower Peninsula, Northwestern Indiana, and
Illinois.
Consumers who purchased the Drinking Water dated December 8 are urged to

Plus Products, Inc. of Bellingham, WA, is recalling its 7 oz. containers of "Smoked
Wild Salmon Spread" because they may contain undeclared egg yolk. People
who have allergies to egg yolk run the risk of serious or life-threatening
allergic reaction if they consume these products.
The
recalled "Smoked Wild Salmon Spread" was distributed in northwest
Washington state retail foods markets. The product comes in a 7 ounce clear
plastic container with a white lid with the label on top and an expiration date
on the side of the carton. All expiration dates are included. The recall was
initiated after it was discovered that the egg yolk-containing product was
distributed in packaging that did not reveal the presence of egg yolk.
Subsequent investigation indicates the problem was caused by a temporary
breakdown in the company's production and packaging processes. Production of
the product has been suspended until FDA and the company are certain that the
problem has been corrected.

Villarreal
Distributors of San Antonio, Texas, is recalling over 20,000 candy containing clay
pots of JARRITOS brand Tamarindo candy because the candy has been found to
contain excessive levels of lead. Lead is very toxic and dangerous to humans,
especially children, women of childbearing age, pregnant women and their unborn
children. Although people with lead in their blood often do not exhibit the
symptoms of lead toxicity, such symptoms include the following: stomach aches,
colic, nausea, vomiting, abnormal irritability, and insomnia. Lead can also
permanently damage the central nervous system, resulting in learning
difficulties in school children as well as cause other long-term health
problems.
The
JARRITOS brand Tamarindo candy was distributed in IL, OH, WI, NJ, MI, IA, MO,
MN, IN, TN, KY, NE, SC, AL and MD, through retail sites, especially ethnic
markets, stores and bakeries that cater to Latinos. The product is imported
from Mexico, manufactured by La Barca II Dulceria in Jalisco, Mexico.
The
product is sold as 12 candy containing clay pots per labeled package. The label
says JARRITOS and Tamarindo 100% Natural with a picture of a boy. Each pot is
also individually packaged in an unlabeled clear plastic bag and some retail
sites may sell the pots individually. Each pot contains approximately 30 grams
of the candy. The recall was the result of a routine sampling program by the
Food and Drug Administration that revealed that the candy had levels as high as
4.60 ppm. It is believed the lead has leached from the clay pots into the
candy.

FDA WARNS CONSUMERS ABOUT IMPORTED JELLY CUP TYPE CANDY
THAT POSES A POTENTIAL CHOKING HAZARD
The
FDA is warning consumers of a potential choking hazard posed by conjac jelly
cups imported from Asia. Some of the candy bears warning labels, suggesting
that it not be eaten by children or the elderly. Other labels warn of a choking
hazard and say to chew the candy thoroughly. Some of the candy is sold with no
warning label at all.
Local
authorities have reported three childrens’ deaths from choking in the United
States associated with conjac jelly candy. There have also been reports of
deaths in other countries. The candy is sold in several fruit flavors. It is
found in small sealed plastic cups that may or may not contain a chunk of
fruit. Each jelly cup is about the size of a single-serve coffee creamer. The
candy is sold in retail stores and on the internet and is sold under a variety
of names.

Warning Against Using Pharma Choice Acetaminophen with

Directorate of Health Canada is warning consumers in Ontario and Quebec not to
use Pharma Choice Acetaminophen with Caffeine and Codeine Caplets, lot number 1C484.
The products were found to contain ASA (acetylsalicylic acid) with Caffeine and
Codeine, which for some people could cause severe, possibly fatal, adverse
reactions.
This product should not be
used if you: (1) are on anticoagulant therapy or have a bleeding disorder
because it could result in serious bleeding; (2) suffer from stomach ulcers
because it could cause serious complications; (3) are allergic to ASA because
it could cause a severe allergic reaction; (4) are an asthmatic sensitive to
ASA because it could result in a serious asthma attack; (5) are pregnant
because it could cause problems/complications; or (6) a child or adolescent
suffering from chicken pox because it could cause a serious adverse reaction.
The product, manufactured by
Vita Health Products Inc., of Winnipeg and distributed by Medis Health and
Pharmaceutical Services Inc., of Saint-Laurent, Quebec, is packed in white,
oblong bottles with the words "Pharma Choice" printed across the
label. The bottles from lot number 1C484 can be identified by their contents,

Publix
Super Markets Inc. is voluntarily recalling Publix Bakery Chocolate Chip Cookies
purchased between Aug. 13and Aug. 20 in Jacksonville division stores.
This
product may contain undeclared pecans and almonds. People who have an allergy
or severe sensitivity to these products run the risk of serious or life
threatening allergic reaction if they consume these products. Publix Spokesman
Lee Brunson said, "Through our internal quality assurance process, we
determined that the product was limited to our Jacksonville Division bakery
departments. The finished cookies may be found in the following varieties: a
three dozen chocolate chip cookie package, three dozen assorted cookie package,
or individual cookies in the sample cookie case. As a precaution, we are
recalling all product purchased between Aug. 13 and Aug. 20 in North Florida,
Southeast Georgia, and in Beaufort and Hilton Head,S.C."
