Discovered: In response to a previous retail microbiological monitoring sample that resulted in positive results for E. coli O157:H7 on 8/12/01, FSIS collected microbiological follow-up samples on 8/21/01, that returned positive results for E. coli O157:H7 on 8/26/01.

Lightlife Foods is voluntarily recalling 7,710 pounds of Smart CutletsTM Seasoned Chick'n after recently being informed by an ingredient supplier that undeclared sulfites are present in one of their ingredients. Because the Smart Cutlets Seasoned Chick'n packages do not list sulfites among the ingredients, two production lots of the product are being recalled nationwide. The product contains undeclared sulfites, and people who are sensitive to sulfites run the risk of serious or life-threatening reaction if they consume this product.

 

Available in retail refrigerator cases nationwide, Smart Cutlets Seasoned Chick'n, UPC #43454-10101, comes in an 8-ounce package, containing two 4-ounce servings. The recall covers product marked "Best by Oct 12 01" and "Best by Oct 14 01," found on the package side panel below the bar code.

The tub lids are marked with a "SHERIFF BLAYLOCK'S BEST CHILI" with "PRODUCT CODE 500," and each lid bears the establishment number "EST. 13516" inside the USDA mark of inspection. Packaging code number "10861" or "20861" is stamped on the body of each tub.

Problem: The product may be contaminated with botulinum toxin.

Discovered: In response to an outbreak / illnesses in the State of Texas, an intact sample of the above-mentioned product was collected on August 30, 2001, and the analysis confirmed the presence of botulinum toxin on September 02, 2001, FSIS is continuing its investigation.

Winn-Dixie spokesman Mickey Clerc said, "Through our internal quality assurance process, we determined that some peanut butter was used instead of caramel. As a precaution, we are recalling all product produced in that batch."

This product comes in one-half gallon containers of Prestige Caramel Coyote Ice Cream which are marked with a three line code date of JAN 24 2002 / (a four digit time code) / 12-356 found above the UPC Bar Code #21140 27053. The company believes that only a small portion of the production run may contain peanut butter. Garelick Farms of Massachusetts, announced today that it is voluntarily recalling one-half gallon round containers of Garelick Farms Mint Cookie ice cream. This product may contain peanuts or pieces of peanuts. Individuals with allergies to peanuts run the risk of a serious or life threatening reaction if they consume this product. Garelick Farms initiated the recall after one peanut was discovered in a bag of cookie crunch, which is blended into the mint ice cream. No illnesses or allergic reactions have been reported.

The ice cream was manufactured by Brigham’s Ice Cream Company at its plant located in Arlington, Massachusetts and was packaged under the Garelick Farms label. The product may have been distributed in stores in Massachusetts, Rhode Island, Connecticut, Maine, New Hampshire, and Vermont.

The product comes in round half-gallon containers of Garelick Farms Mint Cookie ice cream (UPC 30255-66116), which are marked with a code date of “072402.” The company believes 1,000 of the half-gallon containers of the ice cream from the production run were distributed to stores. Delivery truck drivers, distributors, and stores have been alerted and recovery of the product is already underway.

Consumers with half-gallon round containers of Garelick Farms Mint Cookie ice cream marked with the affected code date are urged to return them to the place of purchase for a full refund. Ghio Seafood Products of San Diego, California is recalling hot smoked salmon distributed by Pacific Shellfish in San Diego during July and August 2001 because it has the potential to be contaminated with listeria monocytogenes, an organism that can cause serious and sometimes-fatal infections in small children, frail or elderly people and others with weakened immune systems. Healthy individuals may only suffer short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea. Listeria infection can also be dangerous to pregnant women.

 

The product was distributed under Pacific Shellfish label and was marked with one of the following three production dates: 7/19/01, 8/9/01, or 8/22/01. The product was packaged was packaged in 6 oz vac-pak portions. Although there have been no reported health problems, return any unused products to where it was purchased.

 

The recall was a result of routine sampling, which revealed the finished product, contained the bacteria in one sample. The FDA and Ghio Seafood are continuing their investigation as to what caused the problem. Customers who have purchased Smoked Salmon vac-pak portions produced by Ghio Seafood with production dates of 7/19/01, 8/9/01 or 8/22/01 should return it to Pacific Seafood for a full refund.

 

 

Global Enterprises is recalling T3, a dietary supplement, because it contains tiratricol, also known as triiodothyroacetic acid or TRIAC, a potent thyroid hormone that may cause serious health consequences including heart attacks and strokes.

MC Retail Foods of Omaha, Neb., is recalling the 12-ounce packages of Marie Callender's Breaded Fish with Macaroni and Cheese and Broccoli Florets Dinner because it may contain undeclared egg whites. People who have an allergy or severe sensitivity to egg whites run the risk of serious or life-threatening allergic reaction if they consume this product. However, the product is safe for persons who are not allergic to egg whites. The product is distributed nationwide through retail stores.

 

The product is packed in a 12-ounce paperboard carton marked with the following production codes: 14 E1206, 14 F1206, 14 G1206, and 14 H1206. The production code is located on the side panel in the consumer information box.

 

The recall was initiated after it was discovered that the product containing egg whites was distributed in packaging that did not reveal the presence of egg whites. Galaxy Nutritional Foods, makers of Veggie® Brand Dairy Alternatives is recalling 458 cases of its 6 ounce packages of Apple Pie flavored Veggie Yogurt because they may possibly contain trace amounts of milk-protein. Nestlé Recalls 120 Cans of Nestlé Carnation Follow-Up Formula, 32-Fluid-Ounce Liquid Ready to Feed

 

Nestlé is recalling 120 cans of Nestlé Carnation Follow-Up Formula, 32-fluid-ounce Liquid Ready to Feed, due to excessive magnesium in these cans. Long term extended use of the product may have the potential to cause severe adverse health effects such as low blood pressure and irregular heart beat. Symptoms to look for in children include poor feeding, unusually pale color and weakness. There have been no reports of illnesses associated with this product. If your child has consumed this product and not had these symptoms, simply discontinue use of these cans.

 

The product was distributed only to Wal*Mart stores only in the following Texas cities: Crockett, Hempstead, Lewisville, Denton, Plano, Garland, Murphy, Corsicana, Sulphur Springs, Humble, The Woodlands, Longview, Katy-Houston, Center, Irving, Bedford, Grand Prairie, Lancaster and Arlington.

 

The recall is limited to 120 cans of Nestlé Carnation Follow-Up Formula, 32-fluid-ounce Ready To Feed with the following production code stamped on the top of cans: 1203EWFR3551. The code appears under the "use by" date, 22 Jan 2003 and may or may not be followed by one additional letter. Consumers are instructed to return any affected cans to the Wal*Mart store where it was purchased. The recall was the result of routine quality assurance sampling by the company which revealed that one batch of the formula had a high magnesium content. This product contains egg yolks that are not listed on the product packaging as an ingredient. The pasta was manufactured by New World Pasta for sale and distribution in Albertson's California stores only.

 

Upon notification, Albertson's acted to immediately remove all affected product from its California stores, however it is possible some customers may have purchased the product prior to the recall. The product affected is packaged in 16 oz. bags marked with UPC number 41163 47343 and code dates of 'Best By Mar 29 03' or 'Best By Apr 27 03'. Consumers with packages of Albertson's Pasta Ribbons marked with the affected UPC number and code date are urged to return them to Albertson's for a full refund. Allergy Alert Issued on Undeclared Whey in Fleischmann's Unsalted Stick Margarine

 

ConAgra Dairy Foods of Downers Grove, Ill., is voluntarily recalling less than 3,000 pounds of Fleischmann's Unsalted Stick Margarine because the product may contain an undeclared dairy ingredient. People who have an allergy or severe sensitivity to dairy products may run the risk of serious or life threatening allergic reaction if they consume this product. Affected product was distributed through retail stores in the following states: Maryland, Mississippi, Vermont, Michigan, Virginia, Pennsylvania, Maine, Alabama, Massachusetts, New Hampshire, Ohio, Kentucky, New Jersey, Connecticut, Tennessee, Rhode Island, New York, Georgia, West Virginia.

                 

This Fleischmann's Unsalted Stick Margarine can be identified with the "best if used by" date on the carton of JAN 31 02 O N. Two allergic reactions have been reported. Other than to dairy sensitive consumers or to consumers on a sodium free diet, the product being recalled is safe for consumption. The product may actually be Fleischmann's Original Stick Margarine that contains whey, a dairy protein. The inner wrap on the individual sticks that reads, “Fleischmann’s Original Margarine,” instead of Unsalted, can easily identify the product.

 

This product is being recalled in cooperation with the Food and Drug Administration (FDA). ConAgra Dairy Foods is concerned about the health of its consumers; therefore, we encourage those individuals who are allergic to dairy products to return the above-coded unsalted stick margarine to the store where purchased for a full refund.
